We currently have an excellent opportunity for a Care Coordinator/Case Manager at a leading children's hospital that was named one of the top ten children''s hospitals by U.S. News & World Report in 2009-2010, and has attained Magnet status, the highest national recognition granted to a hospital or medical center for nursing excellence. The person in this role works in the medical surgical unit, and is responsible for the delivery of care throughout the continuum of care for this patient population in the hospital. They work collaboratively with physicians and staff to ensure the best outcomes for children and their families, using efficient and effective care coordination to successfully move the patient through an acute care episode.

The Care Coordinator/Case Manager at this leading children's hospital will use their skills and experience with case management, care coordination, utilization management, quality data capture and complex home discharge planning in their role. This is a fast-paced environment in a teaching institution. Good computer skills at a level that support familiarity with and navigation of multiple computer-based programs would be a definite plus. This is a Monday-Friday, 8:00 a.m. to 4:30 p.m. position that offers a great salary and excellent benefits plus continuing education assistance with possible relocation opportunities.

Qualifications:

Graduate of an accredited school of nursing with a current RN license to practice professional nursing in the State of Texas, required; current CPR/BLS certification, required; certification in Case Management (CCM) or Accredited Case Management (ACM), preferred; five years of pediatric clinical experience; experience in one or more of the following: case management, utilization review, home health care and/or discharge planning, inpatient pediatrics, emergency pediatrics.
